Ji <br />SEH <br />BLitIthrig <br />lur All of <br />CHANGE ORDER <br />City of Ramsey September 16, 2024 <br />OWNER DATE <br />21-08 <br />OWNERS PROJECT NO <br />Water Treatment Plant Trunk Water Main improvements <br />PROJECT DESCRIPTION <br />The following changes shall be made to the contract documents: <br />Description: <br />4 <br />CHANGE ORDER NO <br />RAMSY 174498 71.50 <br />SEH FILE NO. <br />This change order consists of seven miscellaneous items related to the project. labeled 202403-00 a — g on the <br />attached Northdale summary invoices as summarize below: <br />a. At the intersection at Center St and Bunker Lake Blvd, a fire hydrant was required to be relocated out of <br />the bike trail. Total cost = $11,472.53 <br />On the east side of Ramsey Blvd at 143rd Ave. a 6-inch water service was discovered. The City field staff <br />determined this water service was dead so asked the contractor to remove the service and gate valve <br />and plug the stub. Total cost = $13,893.21 <br />c. A 6-inch sewer service conflict was discovered at 7060 143, so the City field staff directed the riser to be <br />cut clown to avoid the conflict with the new 20r watermain. Total cost = $7,942.55 <br />d. A major storm sewer conflict was discovered at Jasper St and 143d, requiring additional fittings and work <br />to offset the conflict. Addition private fiber lines increase the time to relay the 20" watermain pipe. Total <br />cost = $42,019.15 <br />e. Additional work at 115+18 on 143rd to avoid a 6" sewer service_ Additional curb and gutter had to be <br />removed with the new alignment and avoid the storm sewer and catch basin. A revised plan sheet is <br />attached illustrating this field change. Total cost = $12,165.49 <br />f. Additional wire was needed to connect the Flow Meter to the cabinet due to field changes in the final <br />cabinet location made by City field staff. Total cost = $875.44 <br />g. Installation of 2 temporary mailboxes. Total cost = $630.00 <br />The total cost of a-g is $88,998.37. <br />Purpose of Change Order: <br />The purpoose of this change order is to fully address all miscellaneous field changes that came up during the <br />143rd constructoin. <br />Basis of Cost: i Actual D Estimated <br />Attachments (list supporting documents) <br />• Attached are the appropriate justification invoices for all seven field changes.
