<br />-' <br /> <br />2.03 <br /> <br />A workable record and report system is essential to an effective <br /> <br /> <br />operation and maintenance program. Each time a manhole is inspected, <br /> <br /> <br />the results of that inspection should be recorded. Any maintenance <br /> <br /> <br />performed should be recorded along with the materials and manhours <br /> <br /> <br />required to complete the work. The standard Metropolitan Waste <br /> <br /> <br />control commission record forms should be used for manhole <br /> <br /> <br />inspections. <br /> <br />FORMS AND RECORDS <br /> <br />When sewers are inspected, cleaned or repaired, a brief description of <br /> <br /> <br />the work performed should be recorded in a log book. When sewer work <br /> <br /> <br />is done from a manhole, the manhole should be inspected at that time, <br /> <br /> <br />and the whole job description recorded on standard commission record <br /> <br /> <br />forms. Two suggested maintenance forms are included in the Appendix. <br /> <br /> <br />Copies of report forms shall be submitted to the city on a monthly <br /> <br /> <br />basis concurrent with the monthly invoice. <br /> <br />9 <br /> <br />?7 <br />
