<br />, ' <br />/f--v: P ';, <br />,v- : <br />The plan consultant agrees to: <br /> <br />ATTACHMENT 2 <br /> <br />Attend meetings of the LOGIS Health Care Advisory Board. <br /> <br />provide consulting on benefits issues to member groups. <br /> <br />Negotiate rates and coverage levels on behalf of LOGIS with <br />insurance carriers. <br /> <br />Attend and participate in open enrollments. <br /> <br />Attend employee installation meetings if necessary. <br /> <br />Meet with individual group members of LOGISto explain the <br />operation of the health plans and claims processing. <br /> <br />Answer inquiries of member employers, providers, participating <br />employees, or board members concerning the operational <br />requirements and procedures of the plans. <br /> <br />Regularly advise LOGIS concerning developments and practices <br />in the insurance industry, particularly as they relate to <br />Federal and state legislation and HMO Health Plans. <br /> <br />Assist LOGIS and its counsel in the Review of the plan <br />Documents and all amendments thereafter in accordance with the <br />specifications and subject to the approval of LOGIS, which <br />documents will contain rules of eligibility and coverage, <br />benefit levels, and procedures for administration of the plan <br />in connection with the provision of benefits. <br /> <br />Assure that the Insurance Carriers prepare in a timely fashion <br />and submit to LOGIS cumulative year-to-date financial data for <br />LOGIS' use in preparation of reports required by the Internal <br />Revenue Service, Department of Labor, and any other Federal or <br />state agencies. <br /> <br />Review with LOGIS to assure that amounts paid by the plan's <br />insurance providers have been paid solely in connection with <br />the provision of benefits under the plan. <br /> <br />Develop, in consultation with LOGIS, an orderly procedure for <br />appealing and disposing of disputed claims consistent with the <br />terms and conditions of the plans. <br /> <br />Develop in consultation with LOGIS a procedure to determine a <br />claimant's dispute of continuing eligibility for benefits <br />under the plan by examining information provided by, and, if <br />necessary, consulting with, LOGIS and such other persons who <br />have information concerning the claim. <br /> <br />lX.A'l?4 <br />
