HIPAA Privacy Officer <br />HIPAA Security Officer <br />Violations <br />Effective Date 16. July 22, 2006 <br />References: <br />45 C.F.R. § 164.530, 45 C.F.R. § 164.306. <br />13. The Human Resources Manager is the City's HIPAA <br />Privacy Officer. The HIPAA Privacy Officer is responsible <br />for the development and implementation of the HIPAA <br />policies and procedures relating to the privacy of PHI. <br />The HIPAA Privacy Officer is the contact person for any <br />questions or complaints regarding HIPAA privacy. <br />Questions or concerns about HIPAA rights should be <br />directed to the HIPAA Privacy Officer during regular <br />business office hours Monday through Friday, except <br />holidays at <br />(763) 433 -9821. <br />14. The Information Technology Manager is the City's HIPAA <br />Security Officer. The HIPAA Security Officer is responsible <br />for the development and implementation of the HIPAA <br />policies and procedures relating to the security of ePHI. <br />The HIPAA Security Officer is the contact person for any <br />questions or complaints regarding HIPAA security. <br />Questions or concerns about HIPAA rights should be <br />directed to the HIPAA Security Officer during regular <br />business office hours Monday through Friday, except <br />holidays at (763) 433 -9831. <br />15.
