6 <br />2005 Ramsey Kids Safety Camp <br />Snacks, lunches, and beverages. Are provided for all campers on both days. If your child <br />has dietary restrictions, you may provide a non - refrigerated bag lunch and snacks for your <br />child. Please mark clearly with your child's name. (We will not provide refunds for bag <br />lunches.) <br />Transportation to and from camp. Transportation is the responsibility of the parent. Bikes <br />need to be transported to camp one day for the bike safety portion of the camp (you will be <br />notified in advance of which date). All children will receive new bike helmets and will not ride <br />their bikes at the camp until they have been properly fitted with a helmet. Biking to camp is <br />not recommended due to heavy traffic on Hwy 47. <br />Water Safety. Water safety will be taught at a local pool. This course is taught by <br />professional certified lifeguard staff employed by the pool, and will focus strictly on how to be <br />safe around the water while playing or swimming. This class will NOT teach swimming skills. <br />Please send a swimsuit and towel in a plastic bag which should be clearly marked with your <br />child's name on swimming day. All campers and their counselors will be bussed to the <br />facility and then returned to the camp site afterwards. <br />It is very important that all participants and their families attend the Wednesday <br />evening BBQ & awards ceremony. <br />Questions? In accordance with the MN Government Data Privacy Act, the staff of the Safety Camp informs you that the <br />personal information we are requesting of you and /or your child or guardian on this registration form is now considered private. Private data <br />is available to you and to staff who need to have this information to perform their duties, but not to the public. While you may choose to <br />withhold this data, the consequences could be that the staff may not be able to complete your registration, hence you would not be able to <br />participate in the program, and /or you may not receive updated program information, such as schedule changes. <br />Liability Waiver. In consideration of accepting this entry, I hereby, for myself, my heirs, executors and administrators, waive and release any <br />and all rights and claims for damages I may have against the Ramsey Police and Fire Departments, their employees and agents providing the <br />activity listed herein and their representatives, successors and assigns for any and all injuries suffered by me or my child at any activity <br />sponsored by these groups, unless the negligence of the groups caused the injury. <br />Video /Photography for Public Information Purposes. This safety camp will be video taped and photographed for local access television <br />and printed media and materials. Images of the video and /or photographs may also be used for promotional purposes and for showing at the <br />awards ceremony. By enrolling your child in this camp, you authorize the Safety Camp to use these images for promotion of the camp, at its <br />functions and activities.
