AMENDED <br />J5 performance record and a satisfactory record of law compliance, integrity and business <br />96 ethics. <br />97 <br />98 (c) The firm will pay all craft employees on the project, at a minimum, the applicable wage and <br />99 fringe benefit rates, as established for the classification in which the worker is employed, in <br />100 accordance with the Pennsylvania Prevailing Wage Act. 43 PA. CONS. STAT. § 165-1 et seq , <br />101 as amended or recodif ed. These wages shall also be paid to employees performing any <br />102 custom fabrication work for the project. For purposes of this subsection, custom fabrication <br />103 shall mean the fabrication, assembly modification or other production of non-standard <br />104 goods, or materials, including components, fixtures, or parts thereof, that are fabricated <br />105 assembled or modified offsite, but produced specifically for a project covered by this <br />106 ordinance. This requirement shall include, but not be limited to custom fabrication for <br />107 electrical, plumbing, mechanical, refrigeration, heating, cooling, ventilation, sheet metal or <br />108 other duct systems, mechanical insulation, ornamental iron work or boiler systems. <br />109 Fabrication shall also include the fabrication, assembly or other production of components <br />110 or structures prefabricated to specifications for a particular project covered by this <br />111 ordinance, <br />112 <br />113 § 2-1101.3. Contractor Responsibility Certifications <br />114 (a) As a condition of performing work on a public works contract subject to this ordinance, a <br />115 general contractor, construction manager or other lead or prime contractor seeking award of <br />116 a contract shall submit a Contractor Responsibility Certification as specified herein. <br />117 <br />118 (b) The Contractor Responsibility Certification shall be completed on a form provided by the <br />119 City and reference the project for which a bid is being submitted by name and contract or <br />120 project number. <br />121 <br />122 (c) In the Contractor Responsibility Certification, the construction manager, general contractor <br />123 or other lead or prime contractor shall confirm the following facts regarding its past <br />124 performance and work history and its current qualifications and performance capabilities: <br />
