Please complete all items on the registration form. Mail (or drop off) your registration form & payment to the City Anoka Parks Dept., 641 Jacob Lane, Anoka MN 55303 no later than 4:00 pm on May 15 to qualify for the lottery drawing (according to the residency policy). Registrations received after May 15 will be held and processed in the order they were received on May 20. . Phone-in I walk-in registration begins May 20. RegistraJions will be accepted until each swim session begins or class level fills. Mail in reaistrations will not be accepted after June 5. . Beginning June 6, ALL registrations must be completed at the Aquatic Center in person or by phone. Reaistrations will NOT be acceDted at the Parks DeDt after June 5. . Citizens of the cities of Anoka & Ramsey qualify for RESIDENT RATES. Youth that attend a k-12 school (2008-09 school year) physically located within the city of Anoka will also qualify for the resident rate with proof of attendance (school 10 or report card). Citizens of all other cities pay non- resident rate. . Please refer to the 2009 Swim Lesson Schedule for class times. Additional classes will be added if enrollments are high and staff is available. I Please read class descriptions carefully! Refer to strength & endurance requirements when choosing the class that is best for your child. . If you need assistance determining which swim level is appropriate for your child, your residency status, or the applicable fee, please call 763-576-2980. Please identify your 1 st, 2nd and 3rd time choice (within the same session) for each class level you are registering. Every effort will be made to schedule siblings & car pool groups at the same time if indicated on the registration. . You may register for multiple sessions during the mail-in registration, however, yOU may only reaister for the Saturday session if yOU are not reaisterina for Sessions B. C. or D. . Parents, you are responsible for updating class leyels for pre-registered sessions if your child does not progress as anticipated. Be sure to make a copy of your registration form for your records. Confirmation will only be sent by email if reauested. If you do not haye an email address you may call 763-576-2980 for confirmation after May 21. . Checks, MasterCard and Visa are accepted for mail in registration. If paying by check, please enclose one check payable to the City of Anoka for the total amount due for all registration forms enclosed. . Payment MUST accompany registration. Cash payments accepted in-person only. Registrations will be returned if full payment is not included.
