SUMMARY OF COVERAGE OPTIONS <br />The City of Ramsey offers health and dental insurance to all full -time regular <br />employees and eligible part-time regular employees. The City provides a <br />monthly employer contribution to employees who are enrolled in the City's health <br />insurance plan. The following benefits are the options currently available to <br />employees. <br />Medical Insurance: <br />• The City offers three insurance plans: <br />o Three for Free ($2,000 Deductible with VEBA) <br />o $1500 Deductible with VEBA <br />o $25 Co- pay /100% Hospitalization <br />• Employees can also choose between single and family coverage. <br />• Current carrier: HealthPartners <br />• Network: Open Access <br />• Rate history: see attachment. <br />• The City has utilized plan design changes and wellness education to <br />manage premiums. <br />• The City's HRA/VEBA and Flexible spending programs are administered <br />by Genesis Employee Benefits (America's VEBA Solution). <br />• Employee Assistance Plan (EAP) — The City offers employees EAP <br />services provided by its medical carrier. <br />Dental Insurance: <br />• The City offers single or family coverage to full -time or part-time regular <br />employees. <br />• Current carrier: Delta Dental <br />• Network: DeltaPremier USA and DeltaPreferred Option USA <br />• Rate history: <br />2010 0.0% <br />2009 3.36% <br />2008 9.8% <br />COBRA Administration <br />• The City's COBRA program is administered by CBIZ. <br />10 <br />
