(d) Dates and substance of subsequent followup contacts, (e) Date on which the relocated person was required to move from the pro- perty acquired for the project, Date on which the actual relocation occurred and whether relocation was accomplished with the assistance of the Sponsor, referral to other agen- cies, or without assistance. If the lat- ter, an approximate date for actual relocation is acceptable. Type of tenure before and after re- location, (h) Age and race of displaced persons. (4) For displacements from dwelling: (a) Number in family and number of in- dividuals, (b) Type of property (single detached, multifamily, etc.) , Fair market value of the dwelling or monthly rent being paid, (d) Number of rooms occupied. For relocated buinsesses: (a) Type of business, (b) Whether continued or terminated, (c) If relocated, distance moved (esti- mate acceptable), (6) For relocated farms: (a) Whether continued or terminated, (b) If relocated, distance moved (esti- mate acceptable). b. Moving Expense Records. The Sponsor will maintain records containing the following informa- tion regarding moving expense payments: (1) The date the removal of personal property was accomplished, (2) The location from which and to which the personal property was moved, If the personal property was stored tem- porarily, the location where the property was stored, the duration of such storage and jus- tification for the storage and storage charges, (4) Itemized statement of the costs incurred sup- ported by receipted bills or other evidence of expense, Amount of reimbursement claimed, amount allowed and an explanation of any differ- ences, (5) (3) (5) (f) (g) (c) (6) Data supporting any determination that a business cannot be relocated without a sub- stantial loss of its existing patronage and that it is not part of a commercial enter- prise having at least one other establishment not being acquired by the Sponsor, (7) When an "in lieu of moving" payment is made to a business or farm operation, data showing how the payment was computed, (8) When moving expense payments are made in accordance with a schedule, the data called for in (3) and (4) above need not be maintained. Instead, records showing the basis on which payment was made shall be maintained. c. Replacement Housing Payment Records. The Sponsor will maintain records containing the fol- lowing information regarding replacement housing payments: (1) The date of the Sponsor's receipt of each application for such payments, (2) The date on which each payment was made or the application rejected, (3) Supporting data explaining how the amount of the supplemental payment to which the applicant is entitled was calculated, (4) A copy of the dosing statement to support the purchase price or downpayment, and in- cidental expenses when housing is purchased. A copy of the Truth in Lending Statement provided by the lending or mortgage institu- tion or other data including computations to support the increased interest payment. (6) The individual responsible for determining the amount of the replacement housing pay- ments will place in the file a signed and data statement setting forth: (a) The amount of the replacement housing payments, (b) His understanding that the deter- mined amount is to be used in es- tablishing payments under the terms of a grant agreement for an ADAP/ FAAP project. (c) That he has no direct or indirect present or contemplated personal in- terest in this transaction nor will derive any benefit from the replace- ment housing payment, (5) 33
