(d) Dates and substance of subsequent <br />followup contacts, <br />(e) Date on which the relocated person <br />was required to move from the pro- <br />perty acquired for the project, <br />Date on which the actual relocation <br />occurred and whether relocation was <br />accomplished with the assistance of <br />the Sponsor, referral to other agen- <br />cies, or without assistance. If the lat- <br />ter, an approximate date for actual <br />relocation is acceptable. <br />Type of tenure before and after re- <br />location, <br />(h) Age and race of displaced persons. <br />(4) For displacements from dwelling: <br />(a) Number in family and number of in- <br />dividuals, <br />(b) Type of property (single detached, <br />multifamily, etc.) , <br />Fair market value of the dwelling or <br />monthly rent being paid, <br />(d) Number of rooms occupied. <br />For relocated buinsesses: <br />(a) Type of business, <br />(b) Whether continued or terminated, <br />(c) If relocated, distance moved (esti- <br />mate acceptable), <br />(6) For relocated farms: <br />(a) Whether continued or terminated, <br />(b) If relocated, distance moved (esti- <br />mate acceptable). <br />b. Moving Expense Records. The Sponsor will <br />maintain records containing the following informa- <br />tion regarding moving expense payments: <br />(1) The date the removal of personal property <br />was accomplished, <br />(2) The location from which and to which the <br />personal property was moved, <br />If the personal property was stored tem- <br />porarily, the location where the property was <br />stored, the duration of such storage and jus- <br />tification for the storage and storage <br />charges, <br />(4) Itemized statement of the costs incurred sup- <br />ported by receipted bills or other evidence of <br />expense, <br />Amount of reimbursement claimed, amount <br />allowed and an explanation of any differ- <br />ences, <br />(5) <br />(3) <br />(5) <br />(f) <br />(g) <br />(c) <br />(6) Data supporting any determination that a <br />business cannot be relocated without a sub- <br />stantial loss of its existing patronage and <br />that it is not part of a commercial enter- <br />prise having at least one other establishment <br />not being acquired by the Sponsor, <br />(7) When an "in lieu of moving" payment is <br />made to a business or farm operation, data <br />showing how the payment was computed, <br />(8) When moving expense payments are made <br />in accordance with a schedule, the data <br />called for in (3) and (4) above need not <br />be maintained. Instead, records showing the <br />basis on which payment was made shall be <br />maintained. <br />c. Replacement Housing Payment Records. The <br />Sponsor will maintain records containing the fol- <br />lowing information regarding replacement housing <br />payments: <br />(1) The date of the Sponsor's receipt of each <br />application for such payments, <br />(2) The date on which each payment was made <br />or the application rejected, <br />(3) Supporting data explaining how the amount <br />of the supplemental payment to which the <br />applicant is entitled was calculated, <br />(4) A copy of the dosing statement to support <br />the purchase price or downpayment, and in- <br />cidental expenses when housing is purchased. <br />A copy of the Truth in Lending Statement <br />provided by the lending or mortgage institu- <br />tion or other data including computations <br />to support the increased interest payment. <br />(6) The individual responsible for determining <br />the amount of the replacement housing pay- <br />ments will place in the file a signed and data <br />statement setting forth: <br />(a) The amount of the replacement <br />housing payments, <br />(b) His understanding that the deter- <br />mined amount is to be used in es- <br />tablishing payments under the terms <br />of a grant agreement for an ADAP/ <br />FAAP project. <br />(c) That he has no direct or indirect <br />present or contemplated personal in- <br />terest in this transaction nor will <br />derive any benefit from the replace- <br />ment housing payment, <br />(5) <br />33 <br />
