I <br />I <br />I <br />I <br />I <br />I <br /> <br />Grant Agreement Number HI2-II G00021605 Between the Minnesota Department of Health and Ramsey Police Dep~,rtment. <br /> <br />'II. <br /> <br />IH. <br /> <br />IV. <br /> <br />GRANTEE shall make periodical reports to the STATE about the results of local enforcement activities <br />prior to the end of the grant period (See XIX Other Provision). These reports hall include project <br />implementation, compliance check results, follow-up enforcement activities, sanctions implemented, and a <br />description of other local community enforcement activities. <br /> <br />GRANTEE shall be in contact with a STATE-Authorized Representative a minimum of two (2) times <br />during the grant cycle and shall provide written activity reports in a manner as required by STATE. <br /> <br />GRANTEE shall participate in all meetings, conference calls, and trainin.g sessions provided by STATE or <br />STATE-Authorized Representative pertaining to these duties, including evaluation. <br /> <br />GRANTEE shall receive approval from STATE-Authorized Representative before any paid advertising or <br />marketing is initiated by GRANTEE. <br /> <br />Youth shall b'e_inx;olved in developing and implementing activities'in this contract. <br /> <br />CONSIDERATION AND TERMS OF PAYMENT <br /> <br />Consideration for all services performed by GRANTEE pursuant to this grant agreement shaH be paid by <br />the STATE as follows: <br /> <br />Compensation: forty-five thousand, five hundred and forty-five dollars and ten cents dollars <br />($45,545.10). <br /> <br />Reimbursement for travel and subsistence expenses actually and necessarily incurred by <br />GRANTEE in performance of this grant agreement in an amount not to exceed six hundred and <br />thirty-six dollars ($636.00); provided, that GRANTEE shall be reimbursed for travel and <br />subsistence expensies in the same manner and in no greater amount than provided in the current <br />"Commissioner's Plan" promulgated by the Commissioner of Employee Relations. GRANTEE <br />shall not be reimbt'~rsed for travel and subsistence expense incurred outside the State of Minnesota <br />unless it has received prior written approval for such out of state travel from the STATE. <br /> <br />The total obligation of the STATE for all compensation and reimbursements to GRANTEE shall <br />not exceed forty-five thousand, five hundred and forty-five dollars and- ten cents dollars <br />($45,545.10). <br /> <br />Terms of Payment Payments shall be made by the STATE promptly after GRANTEE'S presentation of <br />invoices for services performed and acceptance of such services by the STATE'S Authorized <br />Representative pursuant to Clause VI. Invoices shall be submitted in a form prescribed by the STATE and <br />according to the following schedule: May 15, 2001, and August 15, 2001. <br /> <br />CONDITIONS OF PAYMENT All services provided by GRANTEE pursuant to this grant agreement shall be <br />performed to the satisfaction of the STATE, as determined at the sole discretion of its Authorized Representative, <br />and in acco'rd with all applicable federal, state, and local laws, ordinances, rules and regulations. GRANTEE shall <br />not receive payment for work found by the STATE to be unsatisfactory, or performed in violation of federal, state <br />or local law, ordinance, rule o,' regulation. <br /> <br />TERMS OF AGREEMENT This grant agreement shall be effective on January 2, 2000, or upon the date that the <br />final required signature is obtained by the STATE, pursuant to Minn. Stat. § 16C.05, Subd. 2, whichever <br />occurs later, and shall remain in effect until June 30, 200[, or until all obligations set forth in this grant agreement <br />have been satisfactorily fulfilled, whichever occurs first. GRANTEE understands that NO work should begin <br />under this grant agreement until ALL required signatures have been obtained, and GRANTEE is notified to begin <br />work by the STATE's Authorized Representative. <br /> <br />Page 2 of 5 Pages <br /> <br />-311- <br /> <br /> <br />
