A written notice that contains <br />minimally the following information: <br /> <br />(a) Verbatim copies of Section ?-505 and ?-~0~ of this <br />Chapter. <br />(b) The name, date of birth, and address of the person <br />to whom the notics is ismued and the name of ~he person's <br />custodial parenZ or guardian if the person is a juvenile. <br />(c) A description of the specific conducz ~ha~ forms the <br />basis for ~he issuance of the notice. <br /> (d) A descripsion of the specific covered premises or <br />portion thereof to which the notice applies. <br /> (e) The period during which the notice is in affect, <br />including ~he da~e of its expiration. . <br />(f) The name, ~itle, address, and telephone numbec of a <br />person wi~h authority to modify, amend, or rescind ~he <br />notice. <br /> (~) The method by which the no,ice was served upon <br />person to wh~m i~ was issued.' <br /> <br /> 7-503 Issuance of Trespass Notice. A property manager or <br />tenant may issue a trespass notice to a person only if zkere is <br />probable cause co believe the person ha~ no more than thirty (30) <br />days before the imsuance of ~hat notice: <br /> <br /> (~) committed an act prohibited by state ~tatu~e or City <br />ordinance while on ~he covered premises; or <br /> o <br /> <br /> (2) violated any ~u!e of conduc~ ~or the covered premises <br />that has been conspicuously posted a~ all public entrances to the <br />c~v~red premises or that the properzy manager or tenan~ has <br />provided to the persmn in writing. <br /> <br />7-504 Coverage of Trespass N~tice. <br /> <br /> (1) If issued by a property manager, a trespass no,ice is <br />effective only a~ to those public ~ommon areas and private areas <br />within 5he property manager'~ exclusive control, except tha~ a <br />trespass notice may also cove~ p~ivate common areas and other <br />private areas provided the tenant or tenants in control 6f such <br />areas have agreed in writing to be precluded ~rom invitin~ onto the <br />premises 'any person to whom a trespass notice has been issued unde~ <br />this Chapter. Such a trespass notice must state fha% the tenant or <br />~enants of the covered premises are precluded from inviting onto <br />the covered premises any person to whom a trespass notice has been <br />issued under this Chapter. <br /> (2) If issued by a tenang, the trespass notice is effective <br />only as to those private common areas and private areas over which <br />the ~enanz has control. <br /> (]) A notice broader in coverage than authorized by this <br />Section shall no~ be invalid, but shall be valid to the extent <br />auChorized by this Section.- <br /> <br />7-505 Prohibited.conduct.. <br /> <br />7.-~00-2 <br /> <br />-19,7- <br /> <br /> <br />
