APPENDIX VI <br />MN/DOT LANDSCAPING PARTNERSHIP PROCESS FLOW DIAGRAM <br /> <br /> Contact Mn/DOT Landscaping Programs Coordinator for General Information (651) 779-5076~'~ <br /> <br />Sponsoring Local Government Unit(LGU) must send a letter or resolution to the Mn/DOT District / Metre Division'~ <br /> Engineer requesting sanction and techni:al assistance to develop a proposed landscaping partnership project. <br /> <br /> Pro.am,, s~"~ <br />oordinator, will be asked to assign a Project Manager to provide design and/or technical assistance to the Communit) <br /> <br />Mn/DOT Project Manager (landscape architect or forester) and District / Metro Division Liaison meet with the Community"~ <br /> to discuss project planning, implementation, and maintenance concerns. ' <br /> <br /> Mn/DOT Project Manager prepares or assists the Community designer in preparing conceptual Plans. District /Metro"~ <br /> ivision Liaison obtains preliminary review for safety and.operational concerns,prior to Plan review with the Community~ . <br /> <br /> Mn/DOT Project Manager, District / Metro Division Liaison, and LGU / Community meet to review the Plan and to~ <br /> identify desired modifications, options, timetables, implementation strategies and phasing. <br /> <br /> Mn/DOT Project Manager finalizes or assists the Community designer in final[zing detailed Plans, Cost Estimates, and'~ <br /> Specifications for review and approval by Mn/DOT and LGU / Community representatives. <br /> <br />Upon Mn/DOT and LGU approval of landscape lqans, the LGU / Authorized Primary Contact Person prepares a "complete~ <br /> Partnership Project Application and submits it in duplicate to the Mn/DOT Landscaping Programs Coordinator. <br /> <br />Landscaping Programs Coordinator screens Application with Project Manager for accuracy / completeness and forwards <br /> recommendation for approval / rejection of Funding with the Application to District / Metro Division Liaison. <br /> <br />ubsequent to District / Metro Division feedback, the Landscaping Programs Coordinator notifies applicants of approv~, <br /> rejection, or deferral of their Application and Mn/DOT funding. . <br /> <br />([the Application and Funding are approved, the Mn/DOT Project Manager will draft a Cooperative A~eement detailin <br />he LGU's procurement, installation, and maintenance responsibilities. The Mn/DOT Municipal Agreements Engineer <br />coordinates execution of the Agreement and encumbrance of District/Metro Division landscape partnership Funds. <br /> <br />Mn/DOT Project Manager provides technical assistance to help the Community organize their efforts, procure quality~ <br /> materials, obtain training, and coordinate safe and successful installation and maintenance of their plantings. <br /> <br />MnfDOT Municipal A~eements Engineer authorizes a Lump Sum Payment of the Approved Funding Amount to the LGU~ <br /> upon receiving copies of the Purchase Orders for Approved Materials specified in the Project Application and Plan. <br /> <br />Mn/DOT Project Manager Evaluates Compliance / Success of Project relative to intent by written report including Correc--'x~ <br /> tive Actions if needed and continues providing technical assistance to Community as necessary. <br /> <br /> Send out Thank You Letters and Certificates of Appreciation. ) <br /> 25 - <br /> <br /> <br />
