! <br />! <br /> <br />Vacation Donation Policy <br />July 1999 <br /> <br />Page 3 of 3 <br /> <br />PROCEDURES FOR REQUESTING/DONATING VACATION LEAVE IN THE EVENT <br />OF MA3OR HEDICAL ILLNESS <br /> <br />1. In the event there is an employee in the organization who is close to exhausting <br /> their leave time and is facing an extended leave for major medical illness, they <br /> should complete a "Vacation Leave Donation Request" form from the <br /> Administrative Services Manager. Completed forms shall be submitted to the City <br /> Administrator. <br />2. Once the request has been reviewed and approved by the City Administrator, <br /> specific information will be sent out to employees informing them of the situation <br /> and providing them with the opportunity to donate vacation to a particular <br /> employee. <br />3. Donations will be received by the Administrative Services Manager and the names <br /> will be put on a list and then forwarded to Payroll, as the time is needed. Hours <br /> given to Payroll will be in the order received, unless the donator is at the maximum <br /> accrual allowed. <br />4. Once time is donated, the donator will not be able to take the hours back. As <br /> leave time is needed, the names will be checked off the list and the leave <br /> transferred. All donations will be kept confidential, even to the recipient.
