'1
!

PRELIMINARY PROJECT REQUEST FOP, M

I. PROJECT NAME: Anoka CIP (Community Integration Program)
II. PROJECT ACTIVITY

A. Briefly describe the project

1. Project Location: Anoka CIP is located at 1130 Lund Boulevard in the Anoka
Enterprise Park, It is the 0nly non-profit organization in the business park with 31 for-profit
businesses

2. Project Activities: Anoka CIP is a newly developed day training and habilitation
program (DTH) specifically designed for people with significant support needs who require
intensive staffing, Program participants are at least 21 years or older and have severe physical,
developmentaI, and sensory, disabilities, are medically fragile, and have challenging behaviors.
A couple of these individuals are 1997 graduates of the Anoka-Hennepin School District, ten are
from Cambridge and Brainerd State Hospitals. and the others are from Rise's Coon Rapids CIP
(Communi .ty Integration Program), Rise is constructing a building which should be completed
by February_ 1998. It will be licensed to serve 45 people but will open initially with 20 people,

Anc~ka CIP was developed to meet a growing need in the coun.ty to serve people with more
significant disabilities. In creating a five-year plan for people with developmental disabilities,
Anoka County identified upcoming graduates needing services and concluded that existing
programs would not be able to accommodate these individuals. The coun .ty, therefore, apprcached
Rise to develop a day training and habilitation pro_re'am like our existing program in Coon Rapids
which serves 100 people with severe disabilities and is at capaci .ty.

Anoka CIP will provide job training and placement, therapy services when needed and Support
services including vocational skills, It will increase relationships with peers, self-sufficiency
through earthed wages, increase self-esteem, personal choice-making, and participation in
c0mmuni _ty integration activities,

The overall goal of Anoka CIP will be to help people become as independent and self-sufficient
as possible. Internal and industry-based work sites will be accessed tO develop basic work skills
and behaviors and may include vocational exploration, work experience, and/or Supported
employment. Developing independent living skills will improve rnobili_ty, communication,
.commur~j .ty integration, social skill development, money m~nagement and domestic skills.

Rise's presence in the Anoka Enterprise Park will benefit both Rise and the businesses located
there, The business park is a prime location for our program because it has gmat potential for job
sites and sub-contract production work, Rise currently does sub-contract work for park tenant
Lurid Industries and has a job site for eight people at First Team Sports. Rise plans to develop a
marketing plan to introduce and acquaint the park businesses with Rise participants' capabili .ty
