What semites are provided? <br />Rise provides job training, employment, transitional housing, and supported living <br />skill services to persons with mental, physical, developmental, learning, sensory, <br />and emotional disabilities. <br /> <br />2. Where do you provide services? <br />Rise has facilities/offices in Spring Lake Park, Coon Rapids, Brooklyn Center, Minnea- <br />polis, St. Paul, Anoka, Bloomington, Cehter."Ci~y', and St. Cloud. <br /> <br />3. Indicate whether or not each physical location is accessible to those with mobility <br /> _ impairments. <br />All Rise offices are accessible to people with mobility impairments. <br /> <br />If any of your sites are inaccessible, what accommodations can you make to offer <br />services to those who cannot access your place of business? <br /> <br />5. How do you communicate notices regarding yourse~ices oremployment? How willyou <br /> communicate wRh those with hearin~ or vision impairmen~O <br /> Information on Rise services or employment are conveyed'to Anoka, Ramsey, Hennepin, <br /> and Chisago Counties, as well as to human service agencies and job fairs. Rise has a <br /> sensory impairment program at its Spring Lake Park headquarters for people who have <br /> dual sensory impairments (vision and hearing). Several of our staff for this program <br /> are deaf and use tactile sign language to communicate with our program participants. <br /> Rise also has a program in St. ~aul, the Minnesota Employment Center (MEC) for people <br /> Who are Deaf or Hard of Hearing.
