What semites are provided? Rise provides job training, employment, transitional housing, and supported living skill services to persons with mental, physical, developmental, learning, sensory, and emotional disabilities. 2. Where do you provide services? Rise has facilities/offices in Spring Lake Park, Coon Rapids, Brooklyn Center, Minnea- polis, St. Paul, Anoka, Bloomington, Cehter."Ci~y', and St. Cloud. 3. Indicate whether or not each physical location is accessible to those with mobility _ impairments. All Rise offices are accessible to people with mobility impairments. If any of your sites are inaccessible, what accommodations can you make to offer services to those who cannot access your place of business? 5. How do you communicate notices regarding yourse~ices oremployment? How willyou communicate wRh those with hearin~ or vision impairmen~O Information on Rise services or employment are conveyed'to Anoka, Ramsey, Hennepin, and Chisago Counties, as well as to human service agencies and job fairs. Rise has a sensory impairment program at its Spring Lake Park headquarters for people who have dual sensory impairments (vision and hearing). Several of our staff for this program are deaf and use tactile sign language to communicate with our program participants. Rise also has a program in St. ~aul, the Minnesota Employment Center (MEC) for people Who are Deaf or Hard of Hearing. Many of their staff members are deaf or hard of hear- ing.
