PRELIMINARY PROJECT REQUEST FORM <br /> <br />I. Project Name: Arc Family Services and Community Awareness <br /> Program <br /> <br />II. Project Activity <br /> <br />A. Briefly describe the project. <br /> <br />1. Project location <br />The Support Group and workshops will be held at Holy Cross <br />Lutheran Church in Ramsey. Other services are offered <br />through the Arc office or staff meet with the people in <br />their homes. <br /> <br />2. Project activities (acquisition, relocation, <br /> construction, demolition, rehabilitation, <br /> administration, etc. <br /> <br />Arc would facilitate an ongoing support group that would <br />meet monthly and hold two workshops of ointerest to <br />families. Parents and the pastor of Holy Cross Lutheran <br />Church approached Arc and expressed interest about holding <br />support groups for parents and guardians of individuals with <br />developmental disabilities. <br /> <br />3. Detail the overall number of persons benefiting. <br />Twenty adults with developmental disabilities <br />Ten to fifteen families will use the Toy Library <br />Fifteen to twenty people for the support groups and workshop <br />The balance is individuals and families requesting <br />information and referral. <br /> <br />4. Detail the number of Ramsey residents benefiting in <br /> the past year(s) and the number of Ramsey residents <br /> projected to benefit in the upcoming year. <br /> <br />Arc provides information and services to approximately 100 <br />people in the City. of Ramsey each year. Arc has provided <br />these services to Anoka county and Ramsey residents for over <br />ten years and would welcome the continued support of the <br />City of Ramsey. <br /> <br /> <br />
