PRELIMINARY PROJECT REQUEST FORM <br /> <br />I. PROJECT NAME: Rise, Inc, 8406 Sunset Road NE, Spring Lake Park, MN 55432 <br />II. PROJECT ACTIVITY <br /> <br />A. Briefly describe the project <br /> <br /> 1. Project Location: Rise is headquartered in Spring Lake Park and operates OUt of <br />12 locations in th? Twin Cities and suburbs including facilities/offices in Coon Rapids. Brooklyn <br />Center, northeast Minneapolis, St. Paul, Bloomington. and Center City.. Rise serves residents of <br />Anoka, Hennepirl, Ramsey, and Chisago Counties, <br /> <br /> 2. Project Activities: Rise provides job training and employment opportunities for over <br />1,700 individuals onnually with mental, physical, and developmental disabilities in 30 programs. <br />We anticipate serving 1,800 people by the end of 1996. Over 90 percent of Rise program <br />participants work in jobs in the community. Most however, do not drive and therefore, depend <br />on either generic kinds of transportation such as, the MTC. Metro Mobility.. Anoka Traveler. car <br />POOls, or Rise transportation. Rise transports only those who cannot access public transportation <br />or have no other options, Twen.ty-four vans are used daily at our facilities in Spring Lake Park, <br />Coon Rapids, BrQoklyn Center, and our new program site in Bloomington. Several of our vans <br />have over 200,000 miles on them and need to be replaced, One of the vans we need to purchase <br />is a 15 passenger yehicle and the other is a van with a chair lift. The addition and replacement of <br />vans is an ongoing need and capital expense for Rise each year. <br /> <br />There are no othgr comparable facilities or organizations providing supported and communiW- <br />based employment services tO Anoka County residents. <br /> <br /> 3. Detail the number of persons benefitting: In 1995 Rise provided job training, <br />employment, transitional housing, and supported living services to 1,703 persons with disabilities. <br />During the last five years the population Rise is serving has increased significantly. To meet this <br />growing need lind increased demand for services, Rise has expanded its programs to serve more <br />people with severe disabilities, <br /> <br />Rise transports over 250 people each day. In 1996 Rise transported four residents of Ramsey and <br />served 9 persons from Ramsey in its programs. <br /> <br /> 4. How are they benefitting? Rise meets the vocational rehabilitation, housing, and <br />supported living service needs of individuals with physical, developmental, sensory, emotional <br />and learning disabilities, mental illness, and traumatic brain injuries. Rise provides job training <br />and employment Opportunities which provide income, a sense of self-worth and connections with <br />others in the c0m~unity. Rise pays a commensurate wage based on industry standards, provides <br />a chance for people with disabilities to better their standard of living, thereby, increasing their <br />independence and self-sufficiency, <br /> <br /> <br />
