This agency attempts to <br /> <br />assist people who are economically disadvantaged or who have other limitations <br /> <br />to become or remain self-sufficient." The Senior Outreach program is a part of <br /> <br />this larger mission by assisting clients in accessing services which help them <br /> <br />remain in their homes in a safer and healthier manner through nutritional, <br />medical, and social programs. <br /> <br />Amount of CDBG Funds Requested $ ~P- 7 g b~' o o <br /> <br />Specific Project Description: <br /> <br /> The Senior Outreach Worker identifies and meets with vulnerable and isolated <br /> <br />seniors and disabled adults~ assisting them in obtaining financial~ social, medical, <br />and nutritional services. These often include food programs~ Public Health, chore <br />services~ medical assistance~ crisis home repair, and others. Seniors <br />and disabled adults face frequent problems with transportation, physical ~ emotional, <br />or mental disabilities, and often need special assistance with paperwork completion <br />or advocacy. She is able to act as a case manager with clients. <br /> <br /> <br />
