IV. <br /> <br />VI. <br /> <br />PROJECT ACTIVITIES DURING ADDITIONAL YEARS <br /> <br />Project activities will continue to expand as the population <br />increases and needs continue to grow. As the program has <br />continued, the Outreach Worker has discovered that there are <br />more clients needing multiple areas of assistance, and they <br />require additional contacts throughout the year, rather than <br />just one contact. Each year, the Outreach Worker also <br />locates new seniors to assist, through various referrals. <br /> <br />COMMUNITY PROBLEMS THAT PROJECT ADDRESSES <br /> <br />The Senior Outreach Program helps address issues that many <br />low and moderate income isolated seniors and disabled adults <br />face. This type of isolation results in unawareness of <br />services available or lack of ability to gain access to <br />these services. Specific services to which the Outreach <br />Worker most often refers clients include energy assistance, <br />food programs (food stamps, Fare Share, surplus commodities, <br />food shelves), Public Health Nursing, home health care, <br />Anoka County Financial or Social Services, Social Security <br />resources, legal services, and crisis funding from <br />various resources. Seniors and disabled adults face <br />frequent problems with transportation, physical, emotional <br />or mental disabilities, and often need special assistance <br />with paperwork completion or advocacy. The Outreach Worker <br />is able to act as a case manager with clients, and assists <br />them in seeking long-term solutions. <br /> <br />FUNDING SUMMARY <br /> <br />A. Total revenue needed for the Senior Outreach Worker to <br /> work 5.5 hours per week in Ramsey $2785.00. <br /> <br />B. Total CDBG revenue needed from Ramsey is $2785.00. <br /> <br />C. ACCAP continues to work with all communities to solicit <br /> other sources of funding from public and private sources <br /> to provide supplements to CDBG funding. <br /> <br />ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC. <br /> <br /> <br />
