IV. VI. PROJECT ACTIVITIES DURING ADDITIONAL YEARS Project activities will continue to expand as the population increases and needs continue to grow. As the program has continued, the Outreach Worker has discovered that there are more clients needing multiple areas of assistance, and they require additional contacts throughout the year, rather than just one contact. Each year, the Outreach Worker also locates new seniors to assist, through various referrals. COMMUNITY PROBLEMS THAT PROJECT ADDRESSES The Senior Outreach Program helps address issues that many low and moderate income isolated seniors and disabled adults face. This type of isolation results in unawareness of services available or lack of ability to gain access to these services. Specific services to which the Outreach Worker most often refers clients include energy assistance, food programs (food stamps, Fare Share, surplus commodities, food shelves), Public Health Nursing, home health care, Anoka County Financial or Social Services, Social Security resources, legal services, and crisis funding from various resources. Seniors and disabled adults face frequent problems with transportation, physical, emotional or mental disabilities, and often need special assistance with paperwork completion or advocacy. The Outreach Worker is able to act as a case manager with clients, and assists them in seeking long-term solutions. FUNDING SUMMARY A. Total revenue needed for the Senior Outreach Worker to work 5.5 hours per week in Ramsey $2785.00. B. Total CDBG revenue needed from Ramsey is $2785.00. C. ACCAP continues to work with all communities to solicit other sources of funding from public and private sources to provide supplements to CDBG funding. ANOKA COUNTY COMMUNITY ACTION PROGRAM, INC.
