VII. ACTIVITY COST <br /> <br />A proposed budget is enclosed with this request. <br /> <br />VIII. PROJECT AREA <br /> <br />A. The geographical size of project area is the City of <br /> Ramsey. <br /> <br />B. According to the 1990 Census, there are 626 individuals <br /> over the age of 55 in the City of Ramsey. All senior or <br /> disabled individuals and households are eligible for <br /> Outreach service, though the Outreach Worker makes <br /> specific efforts to reach those that are of a low income <br /> status or most vulnerable. <br /> <br />C. N/A <br /> <br />D. N/A <br /> <br />IX. <br /> <br />ACCAP has a goal of having Senior Outreach Workers serving <br />all areas within.Anoka County. The Senior Outreach Program <br />is part of ACCAP's larger mission of assisting people who <br />are economically disadvantaged or who have other limitations <br />to become or remain self-sufficient. "The mission of Anoka <br />County Community Action Program, Inc. is to be a catalyst in <br />creating tools and opportunities for, and in empowering, <br />lower income Anoka County residents to achieve their <br />aspirations and dreams." The Outreach Worker assists <br />clients in accessing services to help them remain in their <br />homes in a safer and healthier manner through nutritional, <br />medical, and social programs. <br /> <br />Ko <br /> <br />PROJECT CONTACT PERSON <br /> <br />Cathey Weidmann, Coordinator of Senior Programs and Volunteer <br />Services (783-4741) is the primary contact person and project <br />coordinator.
