The <br />Outreach Worker provides case management, referral resources, and <br /> <br />Social, medical, financial or nutritional services sought include <br />food program~, medical assistance, energy assistance, Public Health <br />Nursing, crisis intervention, and domestic violence shelters, among <br /> <br />The Outreach Worker meets bi-weekly with the six other Outreach <br />Workers in ~noka County. This provides support, education and <br />information for the entire staff. She also receives daily <br />supervision iand guidance ~rom the Coordinator oX Senior Programs <br />and ¥olunteer Services at ACCAP. The Outreach Worker works closely <br />with area p~oviders, county officials, senior centers and clubs, <br />and community members in order to identify resources and clients. <br /> <br />utilize the Outreach Worker to help identify individuals who could <br />benefit fro~ special assistance such as Christmas Baskets or <br /> <br />services so ithat they may remain in their homes. Host clients <br />served are v~lnerable, isolated or have other difgiculties which <br />make independent access to services impossible. <br /> <br />The anticipated results and goals of this program are to reach as <br /> <br />independent. The success of this program means that there will be <br />fewer premature or unnecessary nursing home admissions, resulting <br />in lower financial and emotional burdens ~or the clients and the <br />entire community. '- <br /> <br />The Outreach Program in Ramsey is vital to helping these <br />individuals Survive and remain healthy and safe in their homes. <br /> <br />clients away~ and may serve only those in a crisis situation. <br />Partial or full funding would allow 'us to serve individuals in a <br />preventative Imanner, as well as those who face crises.
