Project Report and Award

After notification of a successful application, the Commission, and/or Staff, will establish the
reporting requirements. In most cases this will be a brief written re.pon and will require a personal
appearance at a Commission meeting after completion of the project. Documentation, such as
pictures of the various stages, may be desired by the Commission for use in promoting this
program. Upon successful completion of the project and the report, the City will issue a check for
reimbursement.

APPLICATION AND PROCEDURE

There are four primary elements to this apphcation. The fn'st component of the application is the
qualifying criteria. Proposers must answer the qualifying criteria in the affn-mative to be
considered further. For purposes of Qualifying Criteria Statement Number 5, the 1995 volunteer
monetary rate is $7.00 per hour.

The second is a written narrative. This should speak to the project's expected outcomes and
describe how it meets with the qualifying criteria.

The third part of the application is a written response to the project considerations. Desirable
responses to these considerations could be viewed as "bonus points". As with any area of this
application, a particular order does not represent priority or funding preference.

The final, and an important element of this application, is the project presentation before the
Commission. It will be viewed as extremely helpful to visualize the project (or problem) through
photos, sketches, or to have a "sample" on hand of what the Commission's participation will
facilitate. When possible, these exhibits should be provided to the Commission with the
application itself.

The deadline for receiving requests for projects proposed for't995 is 5:00 p.m., January t3, at the
Ramsey Municipal Center, 15153 Nowthen Boulevard N.W., Ramsey, MN. One copy is
sufficient unless materials are not photo copy suitable (such as photographs) in which case nine
copies would be helpful.

Questions or clarification on the materials presented may be directed to:

Park and
Park and
Park and
Park and
Park and
Park and

Recreation Commission Chairman Ron Hetland

Recreation
Recreation
Recreanon
Recreanon
Recreation

Commissioner Todd Cook
Commissioner Jacld Ftorentius

Commissioner James Gookin
Commissioner Jill Johns
Commissioner Teresa Henn

Park and Recreation Commissioner Jay Swokowski ' ',

City Administrator Ryan Schroeder

Park and Recreation Commission Ex Officio Mark Boos

427-t410
427-1410 or 427-1741

Good Luck!!!

4

Page 3
