(Refer to the Rental Assistance section of the scoring <br />worksheet.) <br />Transfer Ownership: <br />If the ownership entity has changed, provide a copy of the assignment, a revised Transfer Agreement (HTC 20) <br />and Notice of Intent to Transfer Ownership (HTC 27), and Release of Information Authorization Form (HTC 17) <br />(See Chapter 3.F & G), and the Transfer of Ownership Fee (See Chapter 9). <br />Partnership Agreement: <br />Provide a copy of the executed final Partnership Agreement. <br />Photographs: <br />Provide clear photographs of completed building(s). <br />Building Map (HTC 28): <br />Provide a completed Building Map for each building. <br />Identity of Interest: <br />A written disclosure as to any and all Identity of Interest parties (see Chapter 3.1 and J). <br />Affirmative Action and Equal Opportunity Forms: <br />It is the policy of the Minnesota Housing Finance Agency to take affirmative action to provide equal opportunity <br />in all of its endeavors. Complete, execute and return the following forms: <br />a. Affirmative Fair Housing Marketing Plan describing the marketing strategies that an owner will use, <br />including, but not limited to special efforts the owner will make to attract persons who are least likely <br />to apply in addition to a broad cross section of the local population without regard to religion, sex, <br />national origin or status as a recipient of public assistance. <br />b. Equal Employment Opportunity Policy Statement. <br />Tenant Selection Plan: <br />Provide a written tenant selection plan describing the tenant selection policy that an owner will use that must be <br />submitted, reviewed and approved by Minnesota Housing prior to the issuance of the 8609. The written tenant <br />selection plan must establish procedures that, at a minimum, meet the following applicable requirements: <br />a. Minimum and Maximum Household Size: While IRS regulations do not specifically address occupancy <br />requirements, Minnesota Housing encourages maximum utilization of space for developments receiving <br />priority for serving large families; therefore: <br />1. The written occupancy policies should set a minimum of at least one person per bedroom and <br />set maximum standards of at least two persons per bedroom. Owners should also comply with <br />state and local laws, regulations and financing requirements (e.g., if Rural Housing Service, Use <br />RHS regulations), and; <br />2. Where two equally qualified households apply for a unit, preference shall be given to the larger <br />household that is most suitable to the unit size; and <br />b. Cooperatively Developed Housing Plan/Agreement to Provide Other Rental Assistance: Minnesota <br />Housing requires a development receiving priority under the Rental Assistance category for entering <br />into a cooperatively developed housing plan/agreement with the local Public Housing Authority or <br />Redevelopment Authority or other similar entity to provide other rental assistance ; therefore: <br />1. The written selection plan between the owner and the local housing authority or other similar <br />entity must include provisions to support and implement the cooperatively developed housing <br />plan/agreement to provide other rental assistance. <br />Smoke Free Building/s*: <br />If applicable, provide the written policy prohibiting smoking in all the units and all common areas within the <br />building/s of the project for the term of the declaration.
