-7- <br /> <br />The licensee shall implement the revised monitoring program prepared by <br />Foth/Van Dyke and dated March 30, 1987. This monitoring plan is amended <br />as follows: <br /> <br />The sampling frequency for wells lB, 6BB, and 11BB need only be once <br />per year. <br /> <br />Wells 2BBB, 29BB, 30BB, and 3BBB should shall be sampled three times <br />per year. <br /> <br />Well 38BB, placed in the tunnel valley south of Well 34BB, should <br />shall be included in the monitoring program. <br /> <br />The list of volatile organics set forth in the plan sho'Jld shall also <br />include 1, 1-dichloroethylene, methylene chloride, <br />1,1,2,2-tetrachloroethane, and 1,1,2-trichloroethane. <br /> <br />eo <br /> <br />Preservation of phenolic samples with CaSO4 should shall be amended <br />to read CuSO4. <br /> <br />The sampling frequency of the private wells, PW 2, PW 92 and PW 93, <br />shall be increased to three times per year. <br /> <br />7_=. ~ Water level measurements shall be taken and recorded, at the time of each <br /> sampling event, from all monitoring wells and piezometers. <br /> <br />CLOSURE/POST CLOSURE FINANCIAL ASSURANCE <br /> <br />~__~. it. The licensee shall comply with the cloSure/post closure plan dated <br /> April 30, 1987. The financial analysis set forth in this plan is amended <br /> as set forth in Table 1. <br /> <br />2~. I£. The licensee shall provide financial assurance for site operation,~' <br /> closure work (as set.forth in Section~V, subsection 5..^, B, C, D, E, F, <br /> G,.' H, Ii J, 'L, and H of the Solid Waste Ordinance), surface drainage <br /> control, and closure period final cover and turf maintenance by <br /> performance bond in the amount of $350,000. The licensee shall maintain <br /> on site at least 60,395 yds. of acceptable final cover soil (36,895 yds. <br /> middle horizon and 23,500 yds. upper horizon) as set forth in the <br /> licensee's correspondence of July 23, 1987. Said soils shall be <br /> protected from erosion and shall not be used except as may be <br /> specifically approved by the Department. <br /> <br />3. IS_ The licensee shall provide up to $1,412,835 of financial assurance <br /> closure period needs not covered by the bond in item 12 above and post <br /> closure period needs as set forth in Section II, subsection 8. B. (1) <br /> of the Solid Waste Ordinance and that this financial assurance be <br /> financed at the rate of $.50/compacted gate yard and $.35/loose or <br /> uncompacted gate yard of solid waste received. This financial assurance <br /> shall be provided and amended quarterly, based upon an estimate <br /> <br /> <br />
