(4) TERM OF PERMITS <br /> <br /> Ail Conditional Use Permits shall be <br />valid for a specified term not exceeding <br />5 years. The permit shall be subject to <br />annual review for conformance with this <br />Ordinance and performance of the permit <br />holder. The' City Council may also <br />require a performance bond, cash escrow, <br />letter of credit or other financial <br />assurance from the landowner or operator <br />to guarantee conformance with these <br />regulations. All property owners and <br />residents within one thousand feet <br />(1000') of the site shall be notified of <br />the annual conditional use permit review. <br /> <br />(5) INFORMATION REOUIRED <br /> <br /> The applicant shall provide to the <br />City three copies of all information <br />submitted to MPCA for preapplication and <br />application requirements for permits <br />outlined in MR 7001. <br /> <br /> The following information shall also <br />be provided by the persons requesting the <br />permit: <br /> <br /> Subd. 1. Name, address and phone <br />number of person requesting the permit. <br /> <br /> Subd. 2.· Exact legal property <br />description and acreage of the site. <br /> <br /> <br />
