For subsequent Plan years, a LOGIS menber who has not been a <br />member of the Group or has not previously dropped membership in <br />the Group may participate in the Group's next Plan Year beginning <br />January 1 of the following year if: <br /> <br />They sign a Participation Resolution on or before August <br />1 of the preceding Plan Year; <br /> <br />They submit necessary marketing information as requested <br />by the Group or its designated representative; and <br /> <br />The Member is accepted by all of the Group's Health/Life <br />providers/carriers. <br /> <br />B. Other Requirements <br /> <br />1. Definitions for purpose of these Procedures <br /> <br />Benefit-earnin9 employee is a full-time employee <br />working an average of 30 scheduled hours or more per <br />week based on employer definition during the plan <br />year. <br /> <br />Benefit-eligible employee is a part-time employee <br />working at least an average of 20 scheduled hours <br />but less than the defined number of hours to be <br />benefit-earning under the employers definition <br />during the plan year. <br /> <br />Retiree is any employee of a Member of the Group who <br />is entitled to coverage under any Retiree program in <br />force according to agreements held by such Members <br />of the Group. <br /> <br />Non benefit-eligible employee is a part-time <br />employee working less than an average of 20 <br />scheduled hours per week during the plan year. <br /> <br />COBRA eligibiles are employees entitled to continue <br />participation subsequent to the federal legislation <br />commonly termed COBRA as in force and/or amended. <br />COBRA continuation does not constitute retirement <br />coverage. <br /> <br />Elected officials of any Member are not to be <br />considered eligible for participation unless the <br />Member has petitioned for and received approval <br />through the LOGIS Health Care and Life Insurance <br />Committee, and only if such arrangements are <br />acceptable to the health care/life insurance <br />providers/carriers. <br /> <br /> <br />
