The Member must honor the LOGIS' Health and Life <br />insurance policy regarding employee eligibility which is <br />as follows: <br /> <br />An employee of a Member is eligible for health and <br />life insurance if he or she is a regular or <br />probational employee and works at least 20 hours per <br />week· <br /> <br />The Member participating in the group, at its own <br />discretion, may establish a more strict definition <br />of eligibility, but not a more lenient definition. <br />Once the Member's definition of eligibility is <br />established, it must be applied consistently within <br />the employee population. All employees meeting the <br />employers definition of eligibility must be allowed <br />to participate. <br /> <br />Retiree coverage is permitted to the extent formal <br />programs or agreements are in effect and each Member <br />must pay 100% of the lowest single medical premium <br />for any Retirees participating in that program after <br />January 1, 1990 unless identified to the carriers. <br />No one over 65 ii eligible for this coverage. <br /> <br />COBRA-eligible employees may be covered to the <br />extent the corresponding active employee is eligible <br />for coverage. COBRA continuees are treated the same <br />as active at works (they are not in a retiree <br />medical plan, even if their Qualifying Event was <br />retirement) <br /> <br />Elected officials may be covered only to the extent <br />approved by the terms in B.l.f above. <br /> <br />f. Non-Benefit eligible employees may not be covered. <br /> <br />The Member must honor the LOGIS Health and Life Insurance <br />policies regarding the Employer's contribution toward <br />health insurance, which is as follows: <br /> <br />At a minimum, the Member must pay 100% of the lowest <br />single premium for health insurance and group term <br />life insurance for all of its benefit-earning <br />employees and at least 50% of the lowest single <br />premium for benefit-eligible employees selecting <br />Med Center or Group Health and 75% of the lowest <br />single premium for benefit-eligible employees <br />selecting Physician's Health Plan. <br /> <br />Each Member participating in the Group, at its own <br />discretion, may establish a more liberal <br />contribution policy, but not a more conservative <br />policy. <br /> <br />Each Member must follow establish <br />contribution/participation rules regarding benefit- <br />earning and benefit-eligible employees. <br /> <br /> <br />
