(f) The taking of unpaid leave of absence by an employee or an employee's spouse; <br />aid <br /> (g) A significant change in the health coverage of an employee or employee's spouse <br />attributable to the spouse's employment. <br /> <br />ARTICLE IV <br /> <br />Benefil~ <br /> <br /> Section 4.1. Health Care and Insurance Reimbursement Accounts. <br />The City shall establish for each participant a Health Care and Insurance Reimbursement Account. <br />Each Health Care and Insurance Reimbursement Account shall initially contain zero dollars <br />($0.00). <br /> <br /> Section 4.2. Increases in Health Care and Insurance Reimbursement <br />Accounts. A participant's Health Care and Insurance Reimbursement Account shall be increased <br />as follows: <br /> <br /> (a) Each pa), period, by the amount of the portion of the participant's pay conversion <br />contribution elected to apply toward Health Care and Insurance Reimbursement Account pursuant <br />to Article III, Section 3.1, provided, however, that in no event shall any participant's Health Care <br />and Insurance Reimbursement Account be increased so as to contain more than $3,000.00 by <br />reason of increases described in this subsection 4.2(a). <br /> <br /> Section 4.3. Decreases in Health Care and Insurance Reimbursement <br />Account. A participant's Health Care and Insurance Reimbursement Account shall be reduced by <br />the amount of any expenses paid to or on behalf of a participant pursuant to Sections 4.4 or 4.6. <br /> <br />Section 4.4. Health Care Reimbursement Benefits. <br /> <br /> (a) Subject to limitations contained in other provisions of this Plan, and to the extent of <br />the amount contained in the participant's Health Care and Insurance Reimbursement Account, a <br />participant who incurs expenses for medical care or health insurance attributable to him/herself, <br />spouse, or dependents shall be entitled to receive from the City full reimbursement under the terms <br />of the Plan for the entire amount of such expenses incurred after the Effective Date of this Plan. <br />An employee who becomes eligible after the Effective Date of this Plan may be enrolled on January <br />1. <br /> <br /> (b) The City shall pay ail qualifying health care expenses incurred under Sections 4.4 <br />and 4.6 to the participant upon the presentation to the City of: <br /> <br />(i) the participant's claim for reimbursement, <br /> <br /> (ii) a written statement from an independent third party (i.e. the medical <br />provider) stating the expenses incurred for which reimbursement is sought and the amount <br />of that expense, and <br /> <br /> 4 <br /> I 2/11/91 <br /> <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />I <br />! <br /> <br /> <br />
