(e) Designate a portion of regular monthly compensation as pay conversion <br />contribution in accordance with the provisions of Article III, Section 3.1. <br /> <br /> Section 2.2. Enrollment Application. As a condition of participation, each <br /> <br />employee who is eligible to be a participant shall execute and deliver to the City a written <br />application signed by the employee in which specifies the desired effective date of participation in <br />the Plan, designates a sum certain amount of regular monthly compensation not to exceed $120.00 <br />per pay period, in $5.00 increments, as a pay conversion contribution as described in Article III, <br />Section 3.1, makes an election as to the disposition of pay conversion contributions, and supplies <br />any other pertinent information as may be reasonably required by the City. <br /> <br /> ARTICLE III <br /> <br />Pay Conversion <br /> <br /> Section 3.1. Pay Conversion. Each participant shall designate a sum certain <br />not to exceed $120.00 per pay period, in $5.00 increments, of regular monthly compensation as a <br />pay conversion contribution. Each participant shall elect, in the manner provided herein: <br /> <br /> (a) To apply the entire pay conversion contribution toward the participant's Health Care <br />and Insurance Reimbursement Account. <br /> <br />(b) Receive the entire pay conversion contribution as regular compensation. <br /> <br /> (c) Apply any portion (designated by the participant) of the pay conversion contribution <br />toward the participant's Health Care and Insurance Reimbursement Account and receive the <br />remaining portion as regular compensation. <br /> <br /> Section 3.2. Election. A participant's initial election as to the disposition of pay <br /> <br />conversion contributions shall be made as part of the participant's Enrollment Application as <br />provided in Article II, Section 2.2 hereof. Thereafter, a participant may change election as to the <br />disposition of pay conversion contributions only before the beginning of the Plan Year. However, <br />a plan may permit a participant to revoke a benefit election after the period of coverage has <br />commenced and to make a new election with respect to the remainder of the period of coverage if <br />both the revocation and new election are on account of and consistent with a change in family <br />status. The following constitute a change in family status: <br /> <br />(a) <br />(b) <br />(c) <br />(d) <br />(e) <br /> <br />Marriage or divorce of an employee; <br />Birth or adoption of a child by an employee; <br />Death of an employee's spouse or dependent; <br />Termination or commencement of employment of an employee's spouse; <br />The switch from part-time to full-time employment status by an employee or an <br /> <br />employee's spouse; <br /> <br />3 <br />12/11/91 <br /> <br /> <br />
