ARTICLE X <br /> <br />Participation Condilions <br /> <br /> Section 11.1 Participation Conditions. As a condition to participation and <br /> <br />receipt of benefits under this Plan, the participant agrees to: <br /> <br /> (a) <br />below. <br /> <br />Furnish to the City the Application to Participate provided for in Section 11.2 <br /> <br />(b) Observe all rules and regulations implementing this Plan. <br /> <br /> (c) Consent to inquiries by the City with respect to any dependent care assistance <br />involved in a claim under this Plan. <br /> <br /> (d) Submit to the City all reports, bills and other information which the City may <br />reasonably require. <br /> <br /> (e) Designate a portion of regular monthly compensation as pay conversion <br />contribution in accordance with the provisions of Article XII, Section 12.1. <br /> <br /> Section 11.2. Enrollment Application. As a condition of participation, each <br /> <br />employee who is eligible to be a participant shall execute and deliver to the City a written <br />application signed in which specifies the desired effective date of participation in the Plan, <br />designates a sum certain, no geater than $190.00 per pay period of regular monthly compensation <br />in $5.00 increments as a pay conversion contribution as described in Article XII, Section 12.1, <br />makes an election as to the disposition of pay conversion contributions, and supplies any other <br />pertinent information as may be reasonably required by the City. <br /> <br /> ARTICLE XII <br /> Pay Conversion <br /> <br /> Section 12.1. Pay Conversion. Each participant shall designate a sum certain, <br />up to $190.00 per pay period, in $5.00 increments of regular monthly compensation as a pay <br />conversion contribution. Each participant shall elect, in the manner provided herein: <br /> <br /> (a) To apply the entire pay conversion contribution toward the participant's Dependent <br />Care Assistance Account, subject to the limitations described in Article XIII, Section 13.5. <br /> <br /> (b) Receive the entire pay conversion contribution as regular compensation. <br /> <br /> (c) Apply any portion (designated by the participant) of the pay conversion contribution <br />toward the participant's Dependent Care Assistance Account and receive the remaining portion as <br />regular compensation. <br /> <br /> 12 <br />1 2/1 1/9 I <br /> <br /> <br />
