Section 12.2. Election. A participant's initial election as to the disposition of pay <br />conversion contributions shall be made as part of the participant's Enrollment Application to <br />Participate as provided in Article XI, Section 11.2 hereof. Thereafter, a participant may change <br />election as to the disposition of pay conversion contributions only before the beginning of the plan <br />year. However, a plan may permit a participant to revoke a benefit election after the period of <br />coverage has commenced and to make a new election with respect to the remainder of the period of <br />coverage if both the revocation and new election are on account of and consistent with a change in <br />family status. The following constitute a change in family status: <br /> <br /> (a) Marriage or divorce of an employee; <br /> (b) Birth or adoption of a child by an employee; <br /> (c) Death of an employee's spouse or dependent; <br /> (d) Termination or commencement of employment of an employee's spouse; <br /> (e) The switch from part-time to full-time employment status by an employee or an <br />employee's spouse; <br /> (f) The taking of unpaid leave of absence by an employee or an employee's spouse: <br />and <br /> (g) A significant change in the health coverage of an employee or employee's spouse <br />attributable to the spouse's employment. <br /> <br />ARTICLE XIII <br /> <br /> Bcnefil$ <br /> <br /> Section 13.1. Dependent Care Assistance Accounts. The City shall <br /> <br />establish for each participant a Dependent Care Assistance Account. Each Dependent Care <br />Assistance Account shall initially contain zero dollars ($0.00). <br /> <br /> Section 13.2. Increases In Dependent Care Assistance Accounts. A <br />participant's Dependent Care Assistance Account shall be increased as follows: <br /> <br /> (a) Each pay period, by the amount of the portion of the participant's pay conversion <br />contribution elected to apply toward Dependent Care Assistance Account pursuant to Article XII, <br />Section 12.1, provided, however, that in no event shall any participant's Dependent Care <br />Assistance Account be increased so as to contain more than $4,940.00 by reason of increases <br />described in this subsection 13.2(a). <br /> <br /> Section 13.3. Decreases in Dependent Care Assistance Account. A <br />participant's Dependent Care Assistance Account shall be reduced by the amount of any amounts <br />paid to or on behalf of a participant p~suant to Section 13.4. <br /> <br />13 <br />12/11/91 <br /> <br /> <br />
