d. Check List: The Contract Vendor will develop a plowing log to be approved by the City's representative and used <br />during the life of the Contract. The Contract Vendor will review and initial the log at the end of each day that snow <br />removal work has been completed. The Contract Vendor shall maintain all logs for review by the Building <br />Maintenance Supervisor. The City reserves the right to make changes to the log during the life of the Contract. <br />BILLING. The Contract Vendor is to submit a separate billing, by invoice, with an itemization of plowing, hauling and <br />sanding to the City of Ramsey at the close of each calendar month. <br />VENDOR WILL NOT CHARGE SALES TAX. The Contract Vendor will not charge sales tax to the City on the purchase <br />orders for the services offered in this Contract. Snow removal services are not taxable per Minnesota Department of <br />Revenue's Sales Tax Fact Sheet No. 112. <br />PRICE SCHEDULE <br />Prices will be listed as a per occurrence price to perform all services requested. Prices will be based on the level <br />of snowfall as measured at The City of Ramsey Public Works Campus <br />DESCRIPTION LINE <br />1. Snow removal services <br />PRODUCT OR SERVICE PRICE <br />1. 1" up to 3" accumulation $ XXXXX/per occurrence <br />2. 3" up to 5" accumulation $ XXXXX /per occurrence <br />3. 5" up to 7" accumulation $ XXXXX /per occurrence <br />4. 7" up to 9" accumulation $ XXXXX /per occurrence <br />5. 9" up to 12" accumulation $ XXXXX /per occurrence <br />6. 12" up to 15" accumulation $ XXXXX /per occurrence <br />7. 15" and over accumulation $ XXXXX /per occurrence <br />8. Hauling away the snow, as needed $XXXXX /per cubic yard <br />This will include removing or hauling away <br />snow piles left by the City when needed <br />The above rates include all costs associated with providing the Contract services.
