(g) Each application must indicate a description and location of the premises at <br />which the applicant will perform massage services. <br />(h) Each application must be accompanied by written authorization for the City, <br />its agents, and employees to seek information and conduct an investigation into <br />the truth of the statements set forth in the application and the qualifications of the <br />applicant for the license. <br />Section 26 -853. Application. <br />(a) No application under Section 26 -851 or 26 -852 shall be granted unless: <br />(1) It is made on a form or forms provided by the City Clerk. <br />(2) It is signed and dated, indicates the name, phone number, and address <br />of the person providing the information, and includes an attestation to the <br />veracity and completeness of the information contained therein. If the <br />applicant is of a natural person, it shall be signed and sworn to. If the <br />applicant is a partnership, it shall be signed and sworn to by one of the <br />partners. If the applicant is a corporation, it shall be signed and sworn by <br />an officer of the corporation. If the applicant is an incorporated association, <br />it shall be signed by the manager or managing officer. <br />(3) The applicant fully provided all requested information, paid the full <br />license fees, and fully cooperated with the City of Ramsey, its agents, and <br />employees in review of the application. <br />(b). Each massage establishment application under Section 26 -851 shall be <br />accompanied by a bank instrument satisfying the following fees: <br />(1) For each initial application, as the City Council may determine <br />(2) For each investigation, whether for an initial or subsequent application, <br />as the City Council may determine. <br />(c) Each massage therapist application under Section 26 -852 shall be <br />accompanied by a bank instrument satisfying the following fees: <br />(1) For each initial application, as the City Council may determine. <br />(2) For each investigation, whether for an initial or subsequent application, <br />as the City Council may determine. <br />(d) No initial massage establishment application under Section 26 -851 shall be <br />granted if: <br />(1) The applicant or any of the massage establishment's owners, <br />employees, or agents has any conviction for an offense relating directly to <br />the person's ability, capacity, or fitness to perform the duties and discharge <br />the responsibilities of the licensed activity. <br />(2) The applicant, within two years prior to the date of application, was <br />denied a massage therapist, massage establishment, or similar licensure or <br />7 <br />
