had such a licensure revoked or permanently suspended, whether such <br />denial, revocation, or permanent suspension occurred in the City of Ramsey <br />or elsewhere. <br />(3) The applicant, if a natural person, is not 18 years of age or older. <br />(4) The application has not been investigated as indicated in subparagraph <br />(0(1) of this section. <br />(5) The premises that the massage establishment will occupy is not <br />inspected as indicated in subparagraph (0(2) of this section. <br />(6) The application is determined to contain intentionally or recklessly <br />false or misleading information. <br />(7) The massage establishment represented in the application cannot meet <br />the safety and sanitary requirements of the City of Ramsey. <br />(e) No initial massage therapist application under Section 26 -852 shall be granted <br />if: <br />(1) The applicant has a conviction for any offense relating directly to the <br />person's ability, capacity, or fitness to perform the duties and discharge the <br />responsibilities of the licensed activity. <br />(2) The applicant, within two years prior to the date of application, was <br />denied a massage therapist, massage establishment, or similar licensure or <br />has had such a licensure revoked or permanently suspended, whether such <br />denial, revocation, or permanent suspension occurred in the City of Ramsey <br />or elsewhere. <br />(3) The applicant is not 18 years of age or older. <br />(4) The application has not been investigated as indicated in subparagraph <br />(0(1) of this section. <br />(5) The application is determined to contain intentionally or recklessly <br />false or misleading information. <br />(0 An applicant must provide all information required to the City Clerk on forms <br />provided by the city clerk. Upon completion of a massage establishment <br />submission process, the City Clerk shall forward an application for a massage <br />establishment license to the Chief of Police, or an individual designated by the <br />Chief of Police, and the building official. Upon completion of a massage therapist <br />submission process, the City Clerk shall forward an application for a massage <br />therapist license to the Chief of Police or an individual designated by the Chief of <br />Police. <br />(1) Upon receipt of a massage therapist application, the Chief of Police <br />shall initiate an investigation of the applicant and other individuals listed in <br />the application, including performing background checks into criminal <br />history, contacting references, and determining, to the extent reasonable, <br />the veracity of the information contained in the application.
