INSTRUCTIONS: This form has been prepared for the use of city clerks in sending Off-Sale retail liquor licenses (and On-Sale liquor licenses of some cities) to the Liquor Control Director for statutory approval; and for filing certificates of On-Sale and Sunday On-Sale retail liquor Licenses on Director's form No. 9011-O&. This form will serve as the clerk's check list and certificate in retail Liquor license proceedings. The clerk will send one copy of this form to the Director with the documents named herein. (Use form 9068-03 for renewal Off-Sale intoxicating liquor License if no changes since last licensed. If there has been a change; that is, change in ownership~ addition of partner; administrator or administratrix to an estate named; change of officers, directors or stockholders in corporation; change of location; or new liquor establishment, then form 9136-03 must be executed.)
