REQUISITES~ <br />YES NO <br /> <br />In order to be considered for funding, a "Yes" answer must apply to each of the following questions: <br /> <br />N/A <br /> <br /> Are the proposed development items eligible basic facilities? <br /> <br /> Have previous grant awards to the local sponsor been satisfactorily closed out/properly <br /> managed? <br /> <br /> Does the project appear in applicant's comprehensive and/or five year action plan? <br /> <br /> Do existing park facilities appear to be adequately maintained? <br /> <br /> Will the proposed athletic facility be used primarily for nonscholsstic play? <br /> <br /> Ia the planned development compatible with the physical characteristics of the site (i.e., <br /> topography, vegetation, soils/drainage)? Requires a minimum score of 19 pts. under site <br /> suitability. <br /> <br />PRI~IIIES= <br /> <br />Athletic facility applications which best address the following issues will rank more competitively than applications <br />whi~qmdo not address them as well. <br /> <br />1. Interorganizational Cooperation (20 points maximum) Total Points <br /> <br /> Does local financing include donations from civic groups, businesses, and/or individuals? Requires <br /> a letter of commitment from each organization.
