Cenlers June 26, 1984 CERTIFIED MAIL P21 4596636 Dear Mr. Bearl: Reference is made to your application for renewal of your license to operate a "cleanfill demolition landfill~ Reference is also made to ou~ correspondence of May 4 and June 7, 1984 concerning this matter. As yet, you have not modified your application, as required by Section VIII, Subsections 4 and 5 of the Solid Waste Ordinance, so that: 1) the facility is not open to the public; 2) the source of the fill material is specifically identified in the application; and 3) the facility operator contracts directly with the source of the fill material. You are hereby notified that, because of the foregoing deficiencies, your license application is denied. You are further advised that if you wish to appeal this matter you must, within five (5) days, request an appeal hearing. This hearing request must be in writing and state the grounds for appeal and be served personally on the Department by midnight of the fifth County working day following service of this notice. I would also like to remind you that your current license will expire on June 30, 1984. Any operation of this facility after June 30, 1984 would be without a license and would be in violation of the Solid Waste Ordinance.
