The <br />cities that h,ve funded us in the past are considering these requests now, <br />,s I assume are you. FLMHC provides mental health services to people <br />need throughout the northern ,etropolitan area and ~urroundin9 region. <br />i~ a non profit, private, community bamed, mental health center. During <br />the past ~ew years, It has needed to turn to multiple funding ~ources <br />order to accomplish It's mid,ion. Family Li~e Mental ~ealth Center <br />requests Sl,O00.O0 fro, the city of Ra,~ey in order to increase the nu,ber <br />of outpatient mental health visits %o %ho~e who have inadequate <br />%o p,y for those ~ervices. <br /> <br />I <br />I <br />I <br />I <br />I <br />I <br /> <br />Established in August of 1978, Family Life Mental Health Center is located <br />in downtown Anoka. In 1985, FLMHC moved in to Its present location in the <br />Old Post Office Building, 300 East Main Street. In 1985, FLMH¢ ~aw 325 new <br />patients and logged 4000 outpatient visits. These visits include <br />individual psychotherapy, group therapy, psychiatric evaluations, <br />psychological assessments, medication evaluation and treatment, and Day <br />Treat.ent. The Day Treatment program is central to the Center since it <br />provides ongoing therapy to people who have a long history of problems. <br />This program emphasizes independence in daily living situations. This <br />program is not offered by any other community mental health program. <br /> <br />I <br />I <br />I <br /> <br />Enclosed are several pieces of information that should help in your <br />evaluation. The first is our brochure describing the services and <br />philosophy of the Center. Family Life Mental Health Center is an approved <br />provider of services to medical assistance (MA) and general assistance <br />medical coverage (GAMC) recipients. FLMHC is approved by the State under <br />Rule 29 for insurance reimbursement. The monies FLMHC requests will be <br />used to help those who fall through these support systems. <br /> <br />Second is the list of the Board of Directors.
