The first sheet <br />show~ the resident~ of your municipality who have been treated during the <br />period of 7/1/85 to 12/20/85. This schedule shows the lois-female ratio, <br />race, age, whether or not the patient is a designated head of household, <br />handicapped (self described), the average household size of those <br />surveyed, and the annual household income by category. None of this <br />information is required to receive treatment, but all patients are asked <br />to fill out this survey at the time of registration. <br /> <br />The second sheet shows the results from the total survey. Family Life <br />Mental Health Center, since 7/1/85, has treated 360 individuals. 55.6~ of <br />those individuals had an annual income of less than S17o700, and 8.6k <br />chose not to respond. 44.7x were the head of household and the average <br />household size was 3.15 with 14.7x not responding to that question. <br /> <br />The next enclosure is a list of the staff at Family Life Mental Heath <br />Center. Dr. Myron Malecha serves as the Medical Director. He is also <br />acting Director at Cambridge State Hospital and is Assistant Director at <br />Anoka State Hospital. In May, 1986, Dr. Robert Baumer, founder of the <br />Center, will be re~oining the staff. In 1984, Dr. Baumer was recognized by <br />the United Way of Minneapolis as the volunteer of the year for his work at <br />the Center. His services are one of the factors for the increases in 1986. <br /> <br />The 1986 budget is presented next to the 1985 budget and t~e 1985 <br />pro3ected year end. There is also a fee schedule to show the basis for <br />professional fee revenues. The projected 1985 show that professional fees <br />were down 9k from budget, However, discounts from negotiated fees, welfare <br />write downs, and other discounts, were down only 3%. FLMHC saw more people <br />than anticipated on a reduced fee basis, Due to adjustments by the 5tate <br />in welfare rei,bursement, FLMHG anticipates a reduction of welfare write <br />downs during 1986. <br /> <br />1985 projected shows a 27N percent discount in fees, up 2N from budget. <br />1986 shows a 19X discount. The CDBG Funds offset these discounts inpart. <br />It is important to note that discounts do not reflect bad debt. As <br />demonstrated in 1985 pro]ections, a 6X excess in revenues is not a safe <br />margin, and that margin is reduced in 1986. <br /> <br />If your city decides not to fund the Center, there will be no limitations <br />of services placed on your low income residents. However, 76~ of the <br />clients seen are from the municipalities from which funds are being <br />sought. In order to ensure the continuation of services, Falily Life <br />Mental Health Center asks that all cities look upon this request as <br />participation in the Center not as an expense. The final attachment lists <br />those who have contributed in the past and those who are being asked to <br />contribute for the period o£ July 1, 1986 to June 30, 1987. <br /> <br /> <br />
