Subdivision 1. Forms used to collect private and Confidential data shall be included in the public document. The con-~ssioner may require respon- sible, authorities to submit copies of the public document ~equired in sub- division 1, and may request ~dditional info~ation relevant Io data collection practices, policies and procedures. .Subd. 3.
