C. "Document" or "documents" means any written, printed, <br /> typed or other graphic matter of any kind or nature, and all <br /> mechanical, electrical and magnetic recordings, whether an original, <br /> a copy, or a non-identical, copy, however reproduced or altered, <br /> including but not limited to papers, letters, correspondence, <br /> telegrams, inter-office or intra-office communications, memoranda, <br /> notes, notations, notebooks, reports, or records, accounting books <br /> or records, minutes of meetings, contracts, invoices, instructions, <br /> specifications, schedules, tables, charts, transcripts, obligations, <br /> scrapbooks, diaries, computer tapes or programs, any drafts, <br /> revisions or amendments of the above. <br /> <br /> D. "Identify" or "identity" when used in reference to a <br />document means to state: (a) the type of document, e.g., letter, <br />diary, printout, etc.; (b) the title and date thereof; (c) its <br />author's identity; (d) its addressee's identity; (e) its present <br />location and its custodian's identity; and (f) if the document was <br />but no longer is in your possession, control or custody, what <br />disposition was made of it. <br /> <br /> E. "Identify" or "identity" when used in reference to an <br />individual or person means to state: (a) his full name; (b) his <br />present or last known residential address and telephone nUmber; <br />(c) his present or last known business affiliation and job title; <br />and (d) his present or last known business address and telephone <br />number. <br /> <br />-2- <br /> <br /> <br />
