MEMORANDUM OF UNDERSTANDING <br />BETWEEN THE CITY OF RAMSEY AND LAW <br />ENFORCEMENT LABOR SERVICES -CAPTAINS <br />1) City contributions for 2018 health insurance, as follows: <br />The employer will make the following contributions toward group health insurance <br />coverage for employees enrolled in the City's health plan during 2018. In addition, the <br />City will purchase $20,000 of basic life insurance for full-time regular employees. <br />Total Monthly City Contributions <br />• Employee only (single) City contribution is from $700.00 to $723.50, plus up <br />to an additional $53.42 per month toward the premium <br />• Employee and Children City contribution is $871.60, plus up to an additional <br />$64.04 per month toward the premium <br />• Employee and Spouse City contribution is $907.30, plus_x to an additional <br />$67.26 per month toward the premium Apt <br />• Family City contrib en is $1116.90, .lus u. to an addition .24 per <br />month toward the p <br />Total monthly city contributio or H.S.A. shown below, as <br />follows: <br />• Single a $4000 . cti e $190 per month toward the <br />H' BA or IS.A. <br />• Other - plans'receive $13 ► + per month toward the HRA/VEBA or <br />H.S.A. <br />+ spouse plans will receive $160.00 per <br />mon .rd /V . .A. <br />All fami i s • ceive $1' .00 per month toward the HRA/VEBA or <br />H.S.A. <br />Amount o Increase for _ 118 (over 2017 amounts) - Not to Exceed <br />• Single Premium increased by up to $53.42 <br />• Employee + Children increased by up to $64.04 <br />• Employee + Spouse increased by up to $67.26 <br />• Family increased by up to $83.24 <br />Waiver/Credit in lieu of Coverage -- No Change <br />Note: The rate of premium increase in 2018 is capped at 9%.
