(3) The name and address of any training institutions attended and the dates of such <br />attendance. <br />(4) A certified copy of therapeutic massage training from a U.S. accredited school <br />indicating applicant has completed a minimum of 500 hours. <br />(d) Each application must indicate whether the applicant is licensed or has applied to be <br />licensed in other communities to perform massage services, and, if so, in which community <br />the applicant is licensed or has applied and the status of such license or application. <br />(e) Each application must indicate whether the applicant has previously been denied a license <br />to perform massage services, or has had such a license revoked or suspended, and, if so, <br />the circumstances of such denial, revocation, or suspension. <br />(f) Each application must indicate a description of the services to be provided, including <br />specific techniques and equipment. <br />(g) Each application must indicate a description and location of the premises at which the <br />applicant will perform massage services. <br />(h) Each application must be accompanied by written authorization for the city, its agents, and <br />employees to seek information and conduct an investigation into the truth of the statements <br />set forth in the application and the qualifications of the applicant for the license. <br />(i) Applicant must be affiliated with, employed by, or owns a therapeutic massage <br />establishment licensed by the City, or is exempt from such requirement. <br />(j) Applicant has current insurance coverage of $1,000,000.00 for professional liability in the <br />practice of massage. <br />SECTION 4. EFFECTIVE DATE <br />This Ordinance becomes effective 30 days after its passage and publication, subject to City Charter <br />Section 5.04. <br />SECTION 5. SUMMARY <br />The following is the official summary of proposed changes to Ordinance #21-09, as clearly <br />informing the public of the intent and effect of the Ordinance. <br />Ordinance #21-09 updates and adds the following revisions to City Code, Section 26-852 <br />Therapeutic Massage Therapist: <br />Each application for a massage therapist license must be made to the city clerk, or designated <br />City Official on such forms as the clerk, or designated City Official may make available; <br />
