HF100 NINTH ENGROSSMENT REVISOR BD 110100-9 39.1 (1) the name, address, and date of birth of the applicant; 39.2 (2) the disclosure of ownership and control required under paragraph (b); 39.3 (3) the disclosure of whether the applicant or, if the applicant is a business, any officer, 39.4 director, manager, and general partner of the business has ever filed for bankruptcy; 39.5 (4) the address and legal property description of the business; 39.6 (5) documentation showing legal possession of the premises where the business will 39.7 operate, 39.8 (6) a diagram of the premises, including a security drawing; 39.9 (7) a copy of the security plan; 39.10 (8) proof of trade name registration; 39.11 (9) a copy of the applicant's business plan showing the expected size of the business; 39.12 anticipated growth; the methods of record keeping; the knowledge and experience of the 39.13 applicant and any officer, director, manager, and general partner of the business; the 39.14 environmental plan; and other relevant financial and operational components; 39.15 (10) an attestation signed by a bona fide labor organization stating that the applicant has 39.16 entered into a labor peace agreement; 39.17 (11) certification that the applicant will comply with the requirements of this chapter 39.18 relating to the ownership and operation of a cannabis business; 39.19 (12) identification of one or more controlling persons or managerial employees as agents 39.20 who shall be responsible for dealing with the office on all matters; and 39.21 (13) a statement that the apDlicant agrees to respond to the office's suDnlemental reauests 39.22 for information. 39.23 (b) An applicant must file and update as necessary a disclosure of ownership and control. 39.24 The office by rule shall establish the contents and form of the disclosure. Except as provided 39.25 in paragraph (f), the disclosure shall, at a minimum, include the following: 39.26 (1) the management structure, ownership, and control of the applicant or license holder, 39.27 including the name of each cooperative member, officer, director, manager, general partner 39.28 or business entity; the office or position held by each person; each person's percentage 39.29 ownership interest, if any; and, if the business has a parent company, the name of each 39.30 owner, board member, and officer of the parent company and the owner's, board member's, 39.31 or officer's percentage ownership interest in the parent company and the cannabis business; Article 1 Sec. 14. 39
