The registry verification <br />110.2 must include: <br />110.3 (1) the patient's name and date of birth; <br />110.4 (2) the patient registry number assigned to the patient; and <br />110.5 (3) the name and date of birth of the patient's registered designated caregiver, if any, or <br />110.6 the name of the patient's parent, legal guardian, or spouse if the parent, legal guardian, or <br />110.7 spouse will act as a caregiver. <br />110.8 Subd. 6. Conditions of continued enrollment. As conditions of continued enrollment, <br />l i o.9 a patient must: <br />110.10 (1) continue to receive regularly scheduled treatment for the patient's qualifying medical <br />i i o. i i condition from the patient's healthcare practitioner; and <br />110.12 (2) report changes in the patient's qualifying medical condition to the patient's health <br />110.13 care practitioner. <br />110.14 Subd. 7. Enrollment period. Enrollment in the registry program is valid for one year. <br />110.15 To re -enroll, a patient must submit the information required in subdivision 2 and a patient <br />110.16 who is also a veteran must submit the information required in subdivision 3. <br />110.17 Subd. 8. Allowable delivery methods. A patient in the registry program may receive <br />i i o. i 8 medical cannabis flower and medical cannabinoid Droducts. The office may aDDrove <br />i l o. i 9 additional delivery methods to expand the types of products that qualify as medical <br />110.20 cannabinoid products. <br />110.21 Subd. 9. Registered designated caregiver. (a) The Division of Medical Cannabis must <br />110.22 register a designated caregiver for a patient if the patient requires assistance in administering <br />110.23 medical cannabis flower or medical cannabinoid products or in obtaining medical cannabis <br />110.24 flower, medical cannabinoid products, or medical cannabis paraphernalia from a medical <br />110.25 cannabis retailer. <br />110.26 (b) In order to serve as a designated caregiver, a person must: <br />110.27 (1) be at least: <br />110.28 (i) 18 years of age to obtain or assist with medical cannabinoid products or medical <br />110.29 cannabis paraphernalia; and <br />110.30 (ii) 21 years of age to obtain or assist with medical cannabis flower; <br />Article 1 Sec. 52. 110 <br />
