No multi-use utensils <br />will be used (single service utensils only). <br />Adequate separation of food service materials from other day care <br />supplies will be provided. <br />No changes will be made in the food service, such as change of <br />caterer, type of service equipment, etc., without our prior appro- <br />val of this Department. <br /> <br />Enclosed you will find a partially completed license application which must <br />be completed and returned to us with the appropriate fee and your response <br />to the foregoing. <br /> <br />Commencement of any food service activities or receiving of any food supplies <br />cannot occur until the facility has been inspected and approved by this De- <br />partment.
