91 <br /> <br />e <br /> <br />Copies of all environmental data necessary in considering the <br />issuance of environmental permits by state and locaI agencies; <br />compliance with data requirements of the Environmental Quality <br />Board (EQB). <br /> <br />Long-term care provisions for secured deposition sites and <br />identification of the party responsibie for providing long-term <br />care. <br /> <br />Operations <br /> <br />Contingency plans to provide for continued service to contracted <br />solid or hazardous waste generators and haulers in the event of <br />changes in the market value of recovered products. Plans shall <br />include an identification of alternate means of solid or <br />hazardous waste management, including available capacity, cost, <br />location and access routes. <br /> <br />Publicly supported facilities shall identify the facility, s <br />proposed management structure and submit an organizational chart <br />for plant operations. <br /> <br />Identify clauses or procedures in contracts for the delivery of <br />recovered energy which provide for potential changes in the <br />volume and/or content of the waste stream· <br /> <br />Proposed rules and regulations governing the use of the facility <br />by individual generators and solid or hazardous waste haulers. <br />Such rules and reguIations shall address: <br /> <br />o Hours of operation <br /> <br />o Excluded or prohibited materials <br /> <br /> o A fee schedule or charge structure for use of the facility; <br />5. Plans and procedures for: <br /> <br />0 <br /> <br />0 <br /> <br />Containment and management of stored solid or hazardous <br />waste, recovered resource and processing residues <br /> <br />Facility closure and abandonment <br /> <br />0 <br /> <br />0 <br /> <br />Facility cleanup and maintenance of site appearance <br /> <br />Management of non-recoverable wastes and prohibited <br />materials including ash, pollution control residuals, dirt <br />and hazardous wastes <br /> <br />0 <br /> <br />Recording wastes received and assessing user fees or drop <br />charges. <br /> <br /> <br />
