CC Regular Session <br />Meeting Date: 06/25/2024 <br />Primary Strategic Plan Initiative: Not Applicable <br />5. 13. <br />Information <br />Title <br />Adopt Resolution #24-168 Approving Final Payment to Northwest Asphalt and Maintenance for Improvement <br />Project #24-08, 2024 Crack Seal Improvements. <br />Purpose/Background: <br />Resolution and Pay Request attached. <br />Observations/Alternatives: <br />Issuing final payment will be contingent on receipt of the following items from Northwest Asphalt and <br />Maintenance: <br />1. IC-134 (Contractor's Withholding Affidavit showing compliance with the provisions of Minnesota Statute <br />290.92 requiring withholding state income tax). <br />2. Waiver of Mechanic's Lien Rights (Affidavit that all claims against Northwest Asphalt and Maintenance by <br />reasons of the contract have been fully paid or satisfactorily secured). <br />3.
