<br />No Person may Excavate or Obstruct the Right-of-Way beyond the date or dates specified <br />in the Permit unless such person (i) makes a Supplementary Application for another Right-of- <br />Way Permit before the expiration of the initial Permit, and (ii) a new Permit or Permit extension <br />is granted. <br /> <br />Permits issued under this Chapter shall be conspicuously displayed at all times at the <br />indicated work site and shall be available for inspection by the Department Inspectors and <br />authorized City personnel. <br /> <br />Sec. 2.04. Permit Applications. <br /> <br />Application for a Permit is made to the Director. <br /> <br />Right-of- Way Permit applications shall contain, and will be considered complete only <br />upon compliance with, the requirements of the following provisions: <br /> <br />(a) Registration with the Director (or Department) pursuant to Chapter 1. <br /> <br />(b) Submission of a completed Permit application form, including all required <br />attachments, and scaled drawings showing the location and area of the proposed project and the <br />location of all existing and proposed Equipment. <br /> <br />(c) Payment of all money " due to the City for: <br /> <br />(1) prior Obstructions or Excavations; <br /> <br />(2) any loss, damage, or expense suffered by the City as a result of Applicant's <br />prior Excavations or Obstructions of the Rights-of-Way or any Emergency actions taken by the <br />City; and <br /> <br />(3) system management fees. <br /> <br />(d) When an Excavation Permit is requested for purposes of installing additional <br />Equipment, and the posting of a Removal and Restoration Bond for the additional Equipment is <br />insufficient, the posting of an additional or larger Restoration and Removal Bond for the <br />additional Equipment. <br /> <br />Sec. 2.05. Issuance of Permit; Conditions <br /> <br />If the Director determines that the Applicant has satisfied the requirements of Chapters 1 <br />and 2, the Director may issue a Permit. <br /> <br />17 <br /> <br />-" <br />
