No Person may Excavate or Obstruct the Right-of-Way beyond the date or dates specified in the Permit unless such person (i) makes a Supplementary Application for another Right-of- Way Permit before the expiration of the initial Permit, and (ii) a new Permit or Permit extension is granted. Permits issued under this Chapter shall be conspicuously displayed at all times at the indicated work site and shall be available for inspection by the Department Inspectors and authorized City personnel. Sec. 2.04. Permit Applications. Application for a Permit is made to the Director. Right-of- Way Permit applications shall contain, and will be considered complete only upon compliance with, the requirements of the following provisions: (a) Registration with the Director (or Department) pursuant to Chapter 1. (b) Submission of a completed Permit application form, including all required attachments, and scaled drawings showing the location and area of the proposed project and the location of all existing and proposed Equipment. (c) Payment of all money " due to the City for: (1) prior Obstructions or Excavations; (2) any loss, damage, or expense suffered by the City as a result of Applicant's prior Excavations or Obstructions of the Rights-of-Way or any Emergency actions taken by the City; and (3) system management fees. (d) When an Excavation Permit is requested for purposes of installing additional Equipment, and the posting of a Removal and Restoration Bond for the additional Equipment is insufficient, the posting of an additional or larger Restoration and Removal Bond for the additional Equipment. Sec. 2.05. Issuance of Permit; Conditions If the Director determines that the Applicant has satisfied the requirements of Chapters 1 and 2, the Director may issue a Permit. 17 -"
