Advertising Device Permit <br />Permission is hereby granted for the construction of the device as described in the above application, said <br />device to be in accordance with the regulations as set forth in the Commissioner of Transportation's <br />regulations and the laws of the State of Minnesota. Subject to Local Ordinance. <br />SPECIAL REQUIREMENTS: Device must be erected within 120 days or permit will be voided. Permit <br />number must be affixed to device together with Permittee name. It is expressly understood that this permit is <br />conditioned upon maintenance of the device in its original or to a satisfactory condition. <br />This permit expires on June 30, 20 <br />Authorized MnDOT Signature Date Permit No <br />Application Instructions <br />1. Fill out as much of this form as you can use the computer. Then print out the form to sign and send in. <br />2. A complete set of the forms prepared by the applicant should be furnished to the district office. Under <br />normal circumstances an application should be approved or denied within 30 days from date of receipt <br />or applicant advised of reasons for delay. <br />3. Applicant may be a firm, corporation, individual, government agency or other entity. <br />4. Highway No. means the existing number on the highway markers or the lowest number posted where 2 <br />or more routes are carried simultaneously on one roadway. <br />5. Landmarks mean the distance to the nearest intersection or other well-known landmark. <br />6. Advertising Area means that portion of the advertising face of an advertising device which includes the <br />border and trim thereof, but excludes the base and apron supports and other structural members. <br />7. Distance from center of highway means distance in feet from the centerline (the lane nearest the device <br />on multi -lane highways). <br />8. Right of Way distance at sign site means distance in feet from the centerline (the lane nearest the device <br />on multi -lane highways) to the State's property line.
